Mrs Sarah Davey (f)

Sarah is one of our Care Coordinators. She is responsible for liaising with hospitals regarding the safe discharge of patients to home following an inpatient stay, and ensuring that the correct care packages are in place to help patients remain living independently and safely at home. Her follow up with these patients will generally be for a six week period post-discharge, at which time continued contact may be handed over to our other care coordinator, Mary Soellner.

Mary is the Practice Care Coordinator for patients that may have already been seen and followed up by Sarah Davey. Mary's patients will generally be patients that may be living at home with little or no social contact. Mary is able to visit these patients and sign post them to third party organisations that may be able to help with socially related issues, such as heating in the home, Age UK befriending service, Bobby Van and Wilts Fire and Rescue for safety features in the home.

Mary is also able to introduce patients to local groups that meet socially in the Bedwyn area, to ensure that they are more integrated into the fabric of village life.
